Engage-PD is a community-based physical activity coaching program designed to improve exercise uptake and self-management among people with Parkinson’s disease (PwP), with a specific focus on expanding access in the Harlem and Washington Heights neighborhoods. Building on a five-year partnership with local churches, community organizations, and medical centers, the program provides individualized coaching on exercise and related lifestyle factors such as sleep and diet. Delivered through two to six one-on-one sessions either in person or virtually, coaching is led by trained movement professionals or community health workers embedded in the local community. By leveraging trusted community partnerships and culturally responsive delivery, Engage-PD aims to reduce barriers to care, address disparities in participation, and support sustainable physical activity engagement to improve health outcomes for PwP.
